The University of Melbourne was contracted to undertake a pilot quality assurance program and create a database for the Stanford model of Chronic Conditions Self Management (CCSM) in Australia. The Health education improvement questionnaire (heiQ) was developed to assess satisfaction with a course and to advance the process of quantifying, understanding and defining the impact of the chronic disease self-management program on physical and mental health and health related attitudes and behaviours of the participants.The heiQ consists of 51 questions, organised into a set of eight scales. The heiQ is designed to measure effectiveness of health education programs and to inform health professionals and researchers on the outcomes of health education programs delivered to people with chronic diseases.
