Provision of an Evaluation of the Pathways Home Program

The Department of Health and Ageing commissioned HOI in April 2007 to undertake the evaluation of the Pathways Home Program. The objective of the evaluation was to assess whether the Program achieved its original objectives; how the Program contributed to increasing capacity to provided rehabilitation and step-down services and to improve client access to these services; the effectiveness of program implementation and administration and opportunities for improvement. The key tasks included undertaking a situation analysis; developing an evaluation framework and conducting a stakeholder workshop to refine the framework; conducting a survey of service providers; undertaking 20 case studies and the preparation of a final report.
